9th Grade Application
Deadline Date : February 15, 2013
Conditional acceptance for entering freshman will be based on the first 225 applicants who submit the documents outlined below in person, by Email, or FAX to Silva Magnet and who meet the minimum standards described by February 15, 2013:
1. ___ Packet Checklist (page 1)
2. ___ Completed Student/Parent Information Sheet (page 2)
3. ___ Student/Parent Commitment Form signed and dated by applicant and parent (page 3)
4. ___ 9th grade Course Request Form indicating choice of Core Courses and Electives (page 4)
5. ___ Statement of Interest and UIL Agreement signed and dated by applicant and parent
(page 5)
6. ___ Notification Statement signed and dated by applicant (page 6)
7. ___ Academic Probation Guidelines signed and dated by applicant and parent (page 7)
8. ___ Copy of most recent utility bill (water, gas or electric bill only) with parent’s name and
address for proof of residency in El Paso County OR Notarized Residential Affidavit.
9. ___ A copy of the applicant’s transcripts which include:
___ 7th Final grades with an 85% or higher in Math, Science, Social Studies and
English/Language Arts
___ State of Texas Assessments of Academic Readiness (STAAR™) results for 7th Grade
indicating passing scores in Math and Reading (in-state)
___ Recent standardized test scores indicating a 50th percentile ranking in the areas of
Math and Reading (out-of-state)
___ Fall semester 8th grade Report Card (not Progress Report)
___ Copy of Immunization Records
10. ___ Application for Pupil Transfer Request completed and on file with application.

Final Acceptance to the program will be based on the submission of all items above AND
1. ___ 8th Final grades with an 85% or higher in Math, Science, Social Studies and
English/Language Arts received by July12, 2013

3. ___ Student/Parent Commitment Form signed and dated by applicant and parent (page 3)
